Procedures for Acne

For persistent lesions that are inflamed or unresponsive to medications, some doctors recommend additional methods, including extraction, light therapy, or corticosteroid injections.

How Will I Choose a Treatment Plan?

Your doctor will recommend a treatment based on these factors:

Severity of your acne. Mild acne may respond well to a topical retinoid alone. Moderate acne may respond better to a combination of topical retinoid with an antibiotic or other medication. Severe acne with scarring may need treatment with an oral retinoid (Accutane, Sotret).
Results of previous treatments. Medications may be added in a step-wise fashion, only if previous treatments are found to be ineffective.
Degree of scarring. More aggressive therapies may be started earlier if acne scars have already started developing.
Gender. Some treatments are available only for females, such as birth control pills.

What Factors Make Acne Worse?

Acne lesions can come and go. These factors can cause acne to flare:

Changing hormone levels in women 2 to 7 days before their menstrual period, during pregnancy, or when starting or stopping birth control pills
Oil from skin products (moisturizers or cosmetics) or grease in the workplace (for example, a kitchen with fry vats)
Pressure from sports helmets or equipment, backpacks, tight collars, or tight uniforms
Environmental irritants, such as pollution and high humidity
Squeezing or picking at blemishes
Hard scrubbing of the skin

What Are The Treatment Options for Acne?

Almost all cases of acne can be effectively treated. Treatment goals are to heal existing lesions, stop new lesions from forming, and prevent scarring.

Acne treatments aim to control one or more of the underlying causes of acne. For instance, topical retinoids, such as Differin or Retin A Micro, may help unclog sebaceous glands and keep pores open. Antibiotics may be used to fight the P. acnes bacteria. Accutane or hormonal agents, such as birth control pills, may be used to reduce sebum (oil) production.

Over the counter

Benzoyl peroxide—This is found in many products including Clearasil and Proactiv. It is a mild antibiotic that kills the P. acnes bacteria. It is available in different concentrations. Higher concentrations are more likely to irritate the skin. It does not unclog blocked pores so is not as helpful for reducing whiteheads and blackheads.
Sulfur and salicylic acid—These have some mild ability to break down whiteheads and blackheads.

Prescription

Topical retinoids (Differin, Retin A Micro, Tazorac, tretinoin)—These are among the most effective and commonly used acne medications. Topical retinoids are unique in their ability to unclog swollen pores. They may be used alone for mild acne or combined with other medications for moderate-to-severe acne. They may also be recommended for long-term use, even after the acne is under control, to keep the skin clear.
Topical antibiotics—Antibiotics applied to the skin, such as clindamycin (Clindagel) and erythromycin, kill the P. acnes bacteria that leads to inflammation.

Dermatosis papulosa nigra
The lesions of dermatosis papulosa nigra are dark papules of usually less than 2 mm diameter, that can be diffuse on the malar, preauricular, and forehead skin.

Histologically, there are similarities with seborrheic keratoses.

First described in African-Americans, similar lesions are common in Asians and dark-skinned Caucasians.
Treatment:
Lesions respond well to electrofulguration and to the Q-switched ruby or carbon dioxide lasers.
Treatment must be gentle to avoid post-inflammatory hypopigmentation.

1. What is Fraxel™ Laser Treatment (FLT) and what does it do?
The Fraxel laser treatment is a revolutionary way to reverse the signs of aging. This non-surgical procedure smoothes wrinkles and scars, improves texture, tone, elasticity, color and also minimizes pore size. The result is more youthful, radiant skin with minimal down time. Any area of the body can be treated: face, chest, neck, hands! As of January 2008 we have performed over 1600 FLTs.
2. How is it different than other anti-aging treatments and how does it work?
Fraxel is unlike any other rejuvenating skin treatment. Unlike old ablative lasers which remove the top layer of skin, the FLT procedure affects the skin surface by producing thousands of tiny microscopic wounds which penetrate deep into the dermis. These “microthermal zones” are separated by areas of unaffected tissue. The spared tissue between treatment zones promotes rapid healing of the treated area while stimulating collagen production. Thus, FLT results approach improvements obtained with CO2 laser resurfacing but without the associated side effects and downtime.
3. What other applications can FLT be used for?
FLT has been found to be affective in treating acne scars, surgical scars and traumatic scars.
4. How long does a treatment take and what will I look like afterwards? When will I see results?
Treatments last between 10-80 minutes. Side effects are minimal and typically involve mild-to-moderate swelling and redness which usually subsides within several days. Skin may also appear to have a bronzed appearance for several days. Routine activities can be resumed immediately after treatment. Most patients return to work within 1-3 days. Make-up may be worn the next day if desired. Many patients see improvement in discoloration and texture immediately followed by improvement in wrinkles and scars within months.
5. How many treatments are needed and how often can I be treated?
We recommend at least 3 treatments for optimal results. Four to six treatments may be needed for patients with significant scarring and wrinkles. Treatments are usually performed 4 to 6 weeks apart.
6. How can I maximize my result and maintain them?
Since your skin will continue to age naturally, we recommend maintaining your results by using physician-strength rejuvenation products at home and having periodic FLT annually or semi-annually. Periodic FLT’s will help slow down the aging process and keep your skin younger looking, longer. As always, wear a hat and sunscreen when outdoors.
7. How should I prepare for my treatment?
During your pre-Fraxel consultation visit we will discuss which products are best to use prior to, during and after your treatment. Botox may be recommended prior to FLT to relax facial movement in key areas (such as crows feet, frown lines and forehead creases) allowing for better laser penetration into deeper wrinkles. For two weeks prior avoid sun exposure and self tanner and wear a SPF 30 sunscreen daily.
8. Are there any side effects and risks?
Common temporary side effects include redness, swelling, dryness, fine flaking and itching. Significant risks are minimal and may include pigmentation changes, blistering and crusting. Scarring is extremely rare.
9. Can Fraxel treatments be combined with other rejuvenation treatments?

Cryotherapy uses liquid nitrogen to freeze skin lesions. The liquid nitrogen is often applied with a “cryo gun” which sprays a thin mist onto the skin, resulting in a white, frost-like appearance of the skin. It may also be applied with a cotton-tipped swab or other methods.
The extremely cold temperature kills the cells in the superficial layers of the skin. It may be mildly painful, particularly when applied to the face, but anesthesia is rarely if ever necessary.
A blister may form beneath the treated area, which helps to lift the lesion off the skin. It is a sign of an effective treatment.

A bandage may be used to cover the blister and help prevent the area from becoming infected.
Cryotherapy is most commonly used for removing warts, seborrheic keratoses, actinic keratoses, and other benign lesions.

Insect Bites & Stings

I nsect bites and stings are a very common problem, particularly in the summer months.

Bites and stings can be either venomous or non-venomous. Venomous bites and stings usually lead to more severe symptoms.

Venom is a chemical used by insects to kill their prey or defend themselves from larger animals. In humans, venom can damage the tissues and lead to significant pain and inflammation. For those who are allergic to the venom, a sting can trigger hives or a more severe reaction called anaphylaxis.

Insects with venomous bites or stings include:
Bees, hornets, wasps, and yellow jackets
Fire ants
Scorpions
Spiders
One of the most common venomous stings is a bee sting, as shown here:
The most common insect bites are non-venomous. The insects usually bite the skin in search of blood, and may use chemical to numb the site (to avoid being swatted). These chemicals are usually not painful, but they can lead to intense itching.

Insects with non-venomous bites include:
Mosquitoes
Fleas
Ticks
Lice
Bed bugs, which can cause a skin reaction that looks like this:
Following a bite, the skin can become red, swollen, and itchy. A small bump may appear that is usually less than half inch wide and may take up to two weeks to heal. Each person reacts to each bite differently. Some people can have significant reactions to mosquito or flea bites, particularly children, while others can be bitten and have no reaction.
Complications of Insect Bites
Insect bites can sometimes lead to bacterial infections, such as cellulitis, particularly if the wounds are scratched. Try to avoid scratching and keep the area around the bites clean.
Sometimes just one or two bites can trigger more widespread bumps that make it appear like there were many bites. This is called papular urticaria.
Treatment of Insect Bites
Treatment of insect bites and stings is aimed at alleviating the itching and preventing infection.

Applying an icepack to the bites for 5 to 10 minutes can help reduce itching and swelling. An antihistamine, such as Benadryl, may alleviate itching.

People who are allergic to insect stings may need to use medications, such as epinephrine (Epipen), to avoid anaphylaxis.
Prevention
These simple steps can minimize the risk of being bitten by mosquitoes, ticks and fleas.
Reduce mosquito bites by avoiding being outside during dawn and dusk when mosquitoes are most active.
Avoid wooded busy areas with high grass and leaf litter that attract ticks.
Wear long-sleeved shirts, long pants, socks to cover ankles and hats. Keep shirts tucked in and buttoned up.
Place “bee traps” around outdoor eating areas.
Spray clothing, shoes, and camping gear with permethrin-containing repellants (Permanone). These repel and kill ticks and mosquitoes, even after several washings. Permethrin should not be applied directly to skin.
Although there are many insect repellants on the market, those with 20-35% DEET are most commonly recommended. Products with DEET should not be applied to children under 6 months old.

Acne

Acne is a very common skin problem that shows up as outbreaks of bumps called pimples or zits. These usually appear on the face, neck, back, chest, and shoulders. Acne can be a source of emotional distress, and severe cases can lead to permanent scarring.

What Causes Acne?

Acne begins when the pores in the skin become clogged and can no longer drain sebum (an oil made by the sebaceous glands that protects and moisturizes the skin.) The sebum build-up causes the surrounding hair follicle to swell.

Hair follicles swollen with sebum are called comedones. If the sebum stays beneath the skin, the comedones produce white bumps called whiteheads. If the sebum reaches the surface of the skin, the comedones produce darkened bumps called blackheads. This black discoloration is due to sebum darkening when it is exposed to air. It is not due to dirt. Both whiteheads and blackheads may stay in the skin for a long time.

Bacteria called Propionibacterium acnes (P. acnes) that normally live on the top of the skin can enter the clogged pores and infect the sebum. This causes the skin to become swollen, red, and painful.

Infected sebaceous glands may burst, releasing sebum and bacteria into the surrounding skin, creating additional inflammation. In severe cases, larger nodules and cysts may form in the deeper layers of the skin.

What Are the Different Types of Acne?

Acne can be categorized by its severity:

Mild acne describes a few scattered comedones (whiteheads or blackheads) with minimal inflammation (no pustules).
Moderate acne describes a denser collection of comedones as well as red, inflamed, pus-filled lesions (pustules).

Who Gets Acne?

Anyone can get acne, but it appears most often in teenagers, whose surging levels of androgen (a type of hormone) create larger and more active sebaceous glands. Acne may continue for people in their twenties and thirties, and even women over forty. Acne also appears more commonly in people whose parents had acne.
